Paternity tests work on DNA, and DNA is in every cell of the body that has a nucleus. That gives labs a few realistic options for how they get a sample: a cheek swab, a blood draw, or in less standard situations an alternative source like a hair root or a personal item the person has used. What a paternity test sample actually is
Any cell with a nucleus carries a complete copy of someone's DNA. Cheek cells, white blood cells, and the cells inside a hair follicle all qualify. Modern paternity testing compares up to 28 genetic markers across two profiles to determine the probability that an alleged father is or is not the biological father of a child. The lab cares about getting enough clean DNA. It does not care much about whether the DNA came from a swab on your cheek or a blood draw at a clinic. That is why the sample-type debate matters less than people think. What matters more is using a sample type that suits your situation and following the collection instructions carefully.

The three categories USDC and most other postnatal paternity test providers work with are:
- Buccal (cheek) swab — the standard, used for almost all at-home and legal paternity tests.
- Blood sample — used in clinical settings, mostly when buccal isn't possible.
- Alternative samples — hair roots, used cigarettes, toothbrushes, and other discreet sources, used when one person can't or won't provide a fresh swab.
Why cheek swabs are the default
A cheek swab is non-invasive, fast to collect, and produces DNA results that match the accuracy of a blood sample. Because the collection is easier and the result is equivalent, both home paternity test kits and certified legal tests use buccal swabs almost exclusively. The collection takes about 30 seconds per swab. You rub a sterile cotton or synthetic-fiber swab firmly against the inside of one cheek, repeat on the other side or with a second swab, and let everything air-dry before sealing the envelope. Buccal swabs work for every age. A newborn can be safely swabbed minutes after birth, which matters for families finalizing a birth certificate or paternity affidavit early.
Collection tips that actually affect accuracy
- Don't eat, drink, smoke, or chew gum for 30 minutes before swabbing.
- Use a separate swab for each person. Never let swabs from different people touch.
- Let swabs air-dry completely before sealing them in the provided envelope.
- Label each envelope with the person's full name and role (child, alleged father, mother).
- Don't touch the swab tip with your fingers at any point.
For infants, swab a little longer than the standard time — about 20 to 30 seconds per swab. Newborns produce less saliva than adults, and a longer swab time picks up more cells.
The most common cause of an invalid or delayed result is not the sample type. It is contamination, insufficient drying, or a labeling mix-up at the collection stage.
When blood samples come into play
Historically, blood was the gold standard for DNA analysis because white blood cells pack a lot of DNA into a small sample. That hasn't been the case for a long time. Today, blood is used for paternity testing in two situations: a clinical workflow that already involves a blood draw, or a participant for whom a cheek swab won't work.
The second case is narrow but important.
Blood is unreliable after a recent transfusion or bone marrow transplant
If you have had a recent blood transfusion, your blood carries some donor DNA. If you have had a bone marrow transplant, your blood carries a lot of donor DNA — often more than your own. This condition is called chimerism, and it means a blood-based paternity test on you might read out as though you were the donor rather than yourself.
A cheek swab is the only reliable sample type for people in either situation. Cheek cells are not replaced by donor material the way blood cells are.
If this applies to anyone being tested, tell the lab before you collect anything. Most paternity test providers, USDC included, will switch the sample type at no extra cost when the medical history makes it necessary.
Alternative samples for discreet testing
Sometimes the alleged father or another participant is not available for a cheek swab. They may not know about the test, they may refuse, or they may not be in contact. Labs can usually work with alternative samples in those cases: hair with the root attached, a used cigarette butt, a toothbrush, a chewed piece of gum, or fingernail clippings.
These samples produce lower DNA yields and a higher contamination risk, so they are not the first choice when a swab is possible. They often work, though. Alternative samples are not valid for legal paternity tests. A legal DNA test requires a certified collector to verify identity and witness sample collection, which can't happen with a personal item.
How to choose the right sample for your situation
The decision is usually straightforward once you know what you need:

- Court, child support, immigration, or any other legal purpose. You need a legal test with chain-of-custody collection. - Newborn or infant. A buccal swab from birth onward is safe and effective.
- The alleged father is not available. Ask the lab about alternative samples before you collect anything. Some samples work better than others.
A note on AABB accreditation
AABB accreditation is the quality standard most US courts and government agencies look for when a paternity result needs to hold up legally. The AABB Standards for Relationship Testing Laboratories spell out what an accredited lab has to do — from sample handling and chain-of-custody to statistical reporting. , and our current at-home test results are not delivered as AABB-accredited reports. That is the right product for personal use. It is not the right product for court, child support, immigration, or any other proceeding that asks for an accredited result.
When a legal proceeding is involved, the lab's accreditation matters more than the sample type. A cheek swab from an accredited lab is admissible; a blood draw from an unaccredited lab usually is not.
What I have learned watching families go through this
One pattern shows up over and over. People assume blood tests are more "official" than cheek swabs, and that assumption sends them down a path that is slower, more expensive, and no more accurate.
For postnatal paternity testing, the research on this has been consistent for a long time. A correctly collected cheek swab produces the same result a blood draw would. What actually matters is whether the collection protocol was followed and whether the right product was chosen for the situation.
The two times I tell people to slow down and ask a question first are:
- When someone in the test has a recent transfusion or bone marrow transplant. Sample choice matters here, and the wrong choice will give an unusable result.
- When the test needs to hold up in court. Sample choice is secondary to lab accreditation and chain of custody. A home kit is not the right tool for a courtroom no matter how careful you are with collection.
For everything else — a parent wanting confirmation, a grown child wanting answers, an adopted person trying to understand a family connection — a cheek swab at home, collected carefully, is the right answer.
