The honest answer is that these credentials cover different things. AABB Relationship Testing Accreditation
AABB, formerly the American Association of Blood Banks, is a private non-profit standard-setting body based in Bethesda, Maryland. Its Relationship Testing Program has been the U.S. standard for legally defensible DNA parentage and kinship testing since the mid-1980s. The program grew out of a 1984 request from HHS asking AABB to write uniform standards for state IV-D child-support agencies.
The scope is narrow and specific. AABB accredits labs for paternity testing (with or without the mother's sample), maternity, sibling (full and half), grandparent, aunt and uncle (avuncular), twin zygosity, and complex kinship reconstruction using multiple family members. Ancestry, ethnicity, and genealogy services fall outside the program's scope.
How it is granted. A laboratory applies, pays program fees, and submits its quality-system documentation. AABB reviews written procedures, method-validation records, personnel files, and proficiency-testing history, then sends trained assessors on-site to walk the sample workflow and review case files against the reports they generated. Non-conformances require a corrective action plan. Once findings are resolved, accreditation is granted for two years, with proficiency testing continuing throughout. CAP (College of American Pathologists) Accreditation
The College of American Pathologists is a medical society founded in 1946 and based in Northfield, Illinois. CAP runs one of the largest laboratory accreditation programs in the United States, covering the full range of clinical testing from hematology and chemistry to microbiology, cytology, surgical pathology, and molecular diagnostics.
CAP accreditation is broad. It is designed for any clinical laboratory that runs diagnostic tests on human specimens for medical purposes. The standards address personnel qualifications, quality control, proficiency testing, instrument validation, and documentation. Many hospital labs, reference labs, and commercial clinical labs hold CAP as their primary quality credential.
How it is granted. A lab completes a self-inspection against the applicable CAP checklists, then hosts an on-site inspection by peer inspectors from other CAP-accredited institutions. The inspection is specialty-specific, driven by checklists tailored to the disciplines the lab performs. Unresolved findings require a corrective action plan. Accreditation runs on a two-year cycle with a mid-cycle self-inspection. CAP is recognized by CMS as an accrediting organization for CLIA compliance, which means CAP accreditation can substitute for a direct CLIA inspection in many cases.
When it matters for you. CAP signals strong general laboratory quality. What it does not do is establish that the lab is qualified to perform legally defensible parentage testing. The CAP checklists cover clinical diagnostic testing broadly. They are not the AABB Relationship Testing standard. Courts, child-support agencies, and immigration authorities do not accept CAP as a substitute for AABB when the result has to hold up as parentage evidence. ISO/IEC 17025
ISO 17025 is an international standard published jointly by the International Organization for Standardization and the International Electrotechnical Commission. The current version is ISO/IEC 17025:2017. It applies to testing and calibration laboratories in every industry, from environmental testing to forensic labs, and sets requirements for the general competence of a testing lab.
ISO 17025 is a framework, not a subject-matter specification. It covers document control, personnel competence, equipment calibration, method validation, measurement traceability, sample handling, reporting, and internal audits. Because it is written to be applied across radically different testing disciplines, it does not by itself define what a competent paternity testing method looks like. It defines what a competent laboratory quality system looks like around whatever methods the lab has chosen.
How it is granted. A laboratory seeks accreditation from a body that is itself a signatory to the International Laboratory Accreditation Cooperation (ILAC) Mutual Recognition Arrangement. In the U.S. the two most common are the American Association for Laboratory Accreditation (A2LA) and ANSI National Accreditation Board (ANAB). The lab documents its management system and undergoes an on-site assessment covering documentation, witness of test methods, personnel interviews, and equipment checks. Accreditation is typically granted with annual surveillance visits and a full reassessment on a four-year cycle.
When it matters for you. ISO 17025 on a DNA testing lab is a real quality signal, especially for a lab that also does forensic or identification work. Some AABB-accredited relationship testing labs also hold ISO 17025. What it does not do is substitute for AABB in the U.S. legal-testing context. A U.S. court asked to admit a paternity DNA report is looking for AABB Relationship Testing accreditation, not a generic laboratory-competence standard. Outside the U.S., ISO 17025 carries more weight in some countries' domestic legal frameworks, but that is a separate question.
CLIA (Clinical Laboratory Improvement Amendments)
CLIA is not an accreditation. It is a federal regulatory framework. Passed by Congress in 1988 and administered by the Centers for Medicare and Medicaid Services (CMS), CLIA regulates any U.S. laboratory that performs testing on human specimens for the diagnosis, prevention, or treatment of disease. Every clinical lab operating in the U.S. must hold a CLIA certificate for the complexity level of testing it performs, from waived tests up through high-complexity testing.
How it is granted. A laboratory applies through CMS or through a state agency with CLIA exemption authority (New York and Washington are the two exempt states). Fees are based on complexity level and volume. Inspections are conducted by CMS, a state survey agency, or a CMS-approved accrediting organization such as CAP, The Joint Commission, or COLA. Certificates are issued for two years.
When it matters for you. Here is where consumers get tripped up. Paternity and kinship DNA testing is not diagnostic health testing. It is not performed to diagnose, prevent, or treat disease. Relationship testing sits outside the scope CLIA was designed to regulate. A lab that does relationship testing may still hold a CLIA certificate because it also runs other tests that fall under CLIA. But a CLIA certificate on the wall of a paternity testing lab does not translate to relationship-testing accreditation. If a lab lists CLIA as its primary credential for paternity testing, that is a mismatch worth asking about.
BBB (Better Business Bureau) Accreditation
The Better Business Bureau is a private, non-profit organization founded in 1912. BBB accreditation is a business-conduct program. Businesses that meet BBB standards for advertising honesty, transparency, responsiveness to complaints, and ethical practices can apply and, if approved, are listed as BBB Accredited. BBB also assigns a letter-grade rating from A+ to F.
BBB accreditation has nothing to do with laboratory science. It does not evaluate DNA methodology, chain of custody, statistical calculations, or personnel qualifications. It evaluates how the business advertises, handles complaints, and follows through on its commercial commitments.
How it is granted. A business applies through its local BBB, pays annual dues, and agrees to the BBB Code of Business Practices. The BBB reviews complaint history, government-agency actions, licensing status, and advertising practices. Accreditation is renewed annually.
When it matters for you. BBB accreditation and a solid BBB rating tell you something real about how a company treats its customers. That is useful information when you are handing over a credit card. What BBB does not tell you is anything about the lab science. A DNA lab could have a stellar BBB rating and no AABB accreditation, or an AABB accreditation and a mediocre BBB rating. How to Read a DNA Lab's Credentials

If you are ordering a legal or immigration test. Look for AABB Relationship Testing accreditation, current and covering the specific case type you need. Verify it in the AABB public directory. Everything else on the page is secondary. A lab that lists CAP, ISO 17025, CLIA, and BBB but not AABB is not what a court or USCIS is asking for. If you are ordering a private at-home test. The accreditation question matters less than the analytical practice. Ask about the number of genetic markers analyzed (industry standard is 20 or more, some labs analyze up to 28), whether the lab performs independent second-technician review, and whether the report clearly states probability of paternity for inclusions and exclusions. CAP, ISO 17025, or a good BBB rating are legitimate signals here.
Watch for credential creep. Some sites list every acronym they have ever touched. None of that is wrong to mention, but none substitutes for AABB in the legal-testing context. Read what each credential actually covers, not how many logos are on the page.

Are these credentials permanent once a lab earns them?
No. Every one has a renewal cycle, and any of them can be lost between cycles for cause. That is why "accredited" is a status you verify on the date you care about, not a permanent adjective attached to a lab name.
